About the PHQ-2

The Patient Health Questionnaire-2 (PHQ-2) comprises the first two items of the PHQ-9 — depressed mood and anhedonia — and serves as an ultra-brief first-step screen for depression. It is designed to be highly sensitive: a positive PHQ-2 should be followed by the full PHQ-9 or a clinical interview rather than treated as a diagnosis.

Respondents rate how often each of the two cardinal depressive symptoms has bothered them over the past two weeks, giving a total score from 0 to 6. A score of 3 or more is the standard cut-point for a positive screen.
